Head-hanging CT: an alternative method for evaluating traumatic CSF rhinorrhea.

چکیده
A number of methods have evolved to examine the patient with posttraumatic CSF rhinorrhea. Until the advent of metrizamide CT cisternography, radioisotope cisternography was the predominant method of investigation. Metrizamide cisternography has preempted the use of isotopes because of its more precise ability to localize the site of CSF leakage [1-5]. However, this method, which requires lumbar or cervical instillation of contrast medium (usually with fluoroscopic guidance) and careful tilting of the patient to guide the contrast into the head, can be cumbersome in severely traumatized patients. We have found a simpler method-CT in the headhanging position-using intracranial air already present from CSF leak, which demonstrates the site of dural tear in some patients. This technique may be used as an adjunct to, or replacement for, metrizamide cisternography (Fig. 1). Such a method has not been emphasized in the CT literature.

Endoscopic management of cerebrospinal fluid rhinorrhea from anterior skull base defects
Abstract Background: Over the past 20 years, the minimally invasive endoscopic approach has gained widespread acceptance. The study was performed to evaluate the diagnostic method and the success rate of endoscopically diagnosed and treated CSF rhinorrhea, and also investigations such as leakage site and etiologic factor. Methods: This retrospective CSF leakage management review of ...
متن کاملEndoscopic Repair of CSF Rhinorrhea: An Institutional Experience
Introduction: Endoscopic repair is considered the treatment of choice in cerebrospinal fluid (CSF) rhinorrhea. The aim of our study was to analyze the etiopathogenesis of CSF rhinorrhea, the outcome of treatment and the causes of failure in a developing-country setting. Materials and Methods: A retrospective review of patients treated with endoscopic repair for CSF rhinorrhea at a tertiary ca...
متن کاملEndoscopic Endonasal Repair of Cerebrospinal Fluid Leakage Caused by a Rare Traumatic Clival Fracture
An 89-year-old male presented with cerebrospinal fluid (CSF) rhinorrhea associated with head trauma sustained as a pedestrian in a traffic accident. Computed tomography (CT) showed pneumocephalus and multiple cranial bone fractures, including the clivus. Although the CSF rhinorrhea was treated conservatively for a week, clinical symptoms did not improve and surgical repair was performed. Preope...
متن کاملDelayed presentation of traumatic cerebrospinal fluid rhinorrhea: Case report and literature review
Cerebrospinal fluid (CSF) leak is one of several complications that can occur after traumatic skull base injury. Although most patients present soon after the injury occurs, some can present years later, with resulting morbidity and the need for additional procedures. We present a case of a patient with a sphenoid sinus CSF leak who presented 12 years after a closed head injury that included a ...
متن کاملAtypical Clival Fracture Due to Minor Trauma and Cerebrospinal Fluid Rhinorrhea
In some previous cases of clival bone fracture in patients with severe head injury, the diagnosis of clival bone fracture was relatively complicated. Regarding the incidence of clival fractures, some reports have shown an incidence of between 0.21% and 1.2% among traumatic brain injury victims who are admitted to hospital (1-3). The relationship between the site of the head and the type of frac...
